Abstract: Objective Research on the differences in the poverty status of rural middle-aged and elderly poor people in different regions and the poverty reduction effect of medical security, so as to provide suggestions for the improvement of the targeted health poverty alleviation policy system. Methods The FGT index is used to measure the expenditure-based poverty of the rural poor in different regions of China before and after medical security compensation, and further simulate the poverty reduction effects of different medical security levels. Results The poverty level of rural middle-aged and elderly poor population in different regions is different. The eastern region is higher than the central and western regions; the expenditure-oriented poverty caused by outpatient expenses is higher than the cost of hospitalization and self-treatment. The higher the poverty reduction effect, the better. Conclusion Medical security is of great significance for improving the poverty situation of the poor and narrowing the poverty gap between regions. It is necessary to increase medical security for the middle-aged and elderly poor people in rural areas in the eastern region, and improve outpatient compensation policies.
