Abstract: Rectal cancer is one of the most common gastrointestinal malignancies worldwide, and both its incidence and mortality rates remain among the leading causes in China. Accurate preoperative staging is the foundation for individualized and precision treatment of rectal cancer. However, conventional imaging modalities have certain limitations in the staging of early-stage disease. Endorectal ultrasound (ERUS), with its advantages of portability, real-time imaging, noninvasiveness, and high repeatability, has attracted increasing attention for rectal cancer staging. In recent years, ERUS has evolved into a multimodal and multiparametric assessment framework through integration with contrast-enhanced ultrasound, ultrasound elastography, three-dimensional ultrasound, and microvascular flow imaging, among other techniques. This article reviews the applications of multimodal ERUS in T and N staging, summarizes the clinical value of different ultrasound modalities, and discusses the integration of artificial intelligence and radiomics. The aim is to provide a reference for more precise tumor staging and clinical decision-making in rectal cancer.
