Abstract: Abstract:Patients with chronic heart failure (CHF) complicated by renal dysfunction exhibit persistently elevated immune-inflammatory burden due to uremic toxin accumulation, oxidative stress, and endothelial injury, and their prognosis is worse than that of patients with heart failure alone. The systemic immune-inflammation index (SII) and the systemic inflammation response index (SIRI), derived from routine blood counts, are low-cost and reproducible. Current evidence indicates that elevated SII and SIRI are associated with all-cause death, cardiovascular death, heart failure rehospitalization, thrombotic events, and worsening renal function, and may provide supplementary information for the interpretation of treatment response in the context of sacubitril/valsartan and sodium-glucose cotransporter 2 (SGLT2) inhibitor therapy. However, SII and SIRI lack specificity, as they are susceptible to infection, anemia, and nutritional status; cutoffs and statistical methods vary across studies; and most existing studies are single-center, retrospective designs lacking external validation. At present, they should be regarded as risk-stratification and supplementary assessment tools used in combination with NT-proBNP and estimated glomerular filtration rate, and their exact value requires verification in multicenter, prospective studies.
