Experience of Duan Jian?hui in Treating Knee Osteoarthritis Complicated with Chronic Medial Collateral Ligament Injury Based on the Theory of Simultaneous Regulation of Cold?Heat

    Abstract: Numerous clinical studies have focused on isolated knee osteoarthritis. However, few special summaries of renowned?physician experience are available for knee osteoarthritis complicated with chronic medial collateral ligament injury, a common compound disease involving both tendons and bones. Combining traditional Chinese medicine theories with 30?year clinical practice in orthopedics and traumatology, Professor Duan Jian?hui concludes that the core pathogenesis of this disease is cold?heat complex syndrome, with liver?kidney deficiency as the root cause and intermingled cold stagnation and stasis?heat as the secondary manifestation. According to such pathogenesis, he introduces the TCM theory of simultaneous regulation of cold?heat and establishes a characteristic diagnosis?treatment system featured by “simultaneous regulation of cold?heat, equal attention to tendons and bones, and combined internal?external therapy”. Clinically, the modified classical prescription Guizhi Shaoyao Zhimu Decoction is orally administered to harmonize systemic cold and heat, soften tendons and strengthen bones. The improved ancestral hospital?made plaster is applied topically to dredge local meridian obstruction due to stasis?bi. Health education and standardized knee?joint rehabilitation exercises are adopted as adjuvant interventions, achieving satisfactory clinical efficacy. This therapeutic strategy overcomes the drawbacks of cold?heat imbalance and separated management of tendons and bones existing in conventional therapies. With simple medication and favorable safety profile, it is worthy of clinical promotion in primary?level orthopedics and traumatology.
