Treatment of Metabolic-Related Fatty Liver Fibrosis Based on the Theory of “Stagnation Due to Qi Deficiency”

    Abstract: Professor Xie Jun believes that metabolic-related fatty liver fibrosis falls within the scope of “ji ju” (accumulation and stagnation) in Traditional Chinese Medicine. Its pathogenesis begins with the gradual depletion of congenital kidney essence, deficiency of spleen and stomach qi, and the failure of Earth to nourish Wood. This is followed by the accumulation of body fluids into phlegm and the stasis of blood into stasis, with phlegm and stasis obstructing the liver meridians. Ultimately, this leads to concurrent damage to the liver, spleen, and kidneys, as well as a deficiency of essence, qi, and blood, which aligns with the theory of “stagnation due to qi deficiency.” The treatment principle proposed is “tonifying deficiency, resolving stagnation, and consolidating the root.” For cases of spleen-kidney deficiency where “Earth fails to nourish Wood,” the approach is to strengthen the spleen, tonify the kidneys, nourish the Earth, and nourish Wood, using a modified version of the self-formulated Xiao Ji Decoction. For cases where phlegm and stasis are entangled and stasis obstructs the liver meridians, the approach is to resolve phlegm, remove stasis, drain dampness, and unblock the meridians, using a modified combination of Wen Dan Decoction and Ge Xia Zhu Yu Decoction. For cases involving damage to all three zang organs and deficiency of essence, qi, and blood, the approach is to replenish essence, consolidate the kidneys, strengthen the spleen, and nourish the liver, using a modified formula combining Yiguan Decoction and Zishui Qinggan Decoction.
