Abstract: Objective To compare the specific efficacy of transvaginal uterine sling(TVUS)versus traditional transvaginal mesh(TVM)in the treatment of severe pelvic organ prolapse. Methods A retrospective analysis was conducted on the clinical data of 72 patients with severe pelvic organ prolapse treated at the Anhui Provincial Women and Children Medical Center between September 2021 and February 2025. Results The TVUS group demonstrated superior outcomes compared to the TVM group in terms of operation time, intraoperative blood loss, length of hospital stay, postoperative indwelling catheter time, and preservation of total vaginal length (TVL) (P < 0.05). The TVM group showed better anterior compartment recovery than the TVUS group, and the difference was statistically significant (P < 0.05). Postoperative quality of life questionnaire scores significantly decreased in both groups compared to preoperative scores, with no statistically significant difference between the groups (P > 0.05). Conclusion Both surgical methods are effective in treating severe pelvic organ prolapse. The TVUS procedure is more suitable for patients with multiple comorbidities, those who cannot tolerate long surgeries, and those who attach importance to the preservation of sexual function. The TVM procedure is more appropriate for patients with severe anterior pelvic organ prolapse.
