摘要:目的:有效的医患沟通(Doctor-Patient Communication, DPC)有助于提高患者治疗依从性及治疗效果,DPC亦是医学生的核心能力,尤其是肿瘤学专业医学学生。本研究旨在了解肿瘤学专业医学生DPC的培训现状、需求及态度。方法:对北京协和医学院在读肿瘤学专业医学生进行问卷调查,包括基本信息、DPC教育现状及需求、沟通技能态度量表(Communication Skills Attitude Scale, CSAS),并对结果进行统计分析。结果:192名肿瘤学专业医学生完成了有效问卷调查。63.0%的学生曾接受过专门的DPC培训,接受过专门DPC培训的比例专业型研究生高于学术型学生且差异有统计学意义(P<0.001),分别为73.4%和42.2%。97.9%的学生认为DPC对自身很重要,且不同性别和学位类型医学生在DPC教育需求方面没有差异。CSAS方面,男生在“F2负面信念”“F4评估”“F5自负”三个维度上的得分均高于女生且差异有统计学意义(P=0.035,P=0.049,P=0.004)。专业型研究生在“Q19认为成为一名医生无需良好的沟通技能”问题上的得分低于学术型且差异有统计学意义(P=0.014)。结论:肿瘤学专业医学生对医患沟通的态度积极,对于医患沟通的教学需求亦高,但参与率仍待提升。建议构建与肿瘤学专业医学生密切相关的案例库将教学和临床、理论与实践更好地融合,同时按照医学生类型及需求等进行分层教学。
Abstract: Objective: Effective doctor-patient communication (DPC) is important to improve the treatment compliance and efficacy, and DPC is also a core competency for medical students, especially those majoring in oncology. This study aims to investigate the current status, demands, and attitudes of DPC training for medical students majoring in oncology. Methods: A questionnaire survey was conducted on medical students majoring in oncology at Peking Union Medical College, including basic information, current status and demands for DPC education, and communication skills attitude scale (CSAS). The data were statistically analyzed. Results: 192 medical students majoring in oncology completed the effective questionnaire survey. 63.0% of students received specialized DPC training. The proportion of professional-degree postgraduate students who received specialized DPC training was higher than that of academic-degree students, and the difference was statistically significant (P<0.001), with 73.4% and 42.2% respectively. 97.9% of students believed that DPC was important to themselves, and there was no difference in the educational demands for DPC among medical students of different genders and degree types. In terms of the CSAS, male students scored significantly higher than female students in the three dimensions of F2 Negative Beliefs F4 Assessment and F5 Arrogance and the differences were statistically significant (P=0.035, P=0.049, P=0.004). Professional-degree postgraduate students scored lower than academic-degree students on the question Q19 I dont need good communication skills to be a doctor and the difference was statistically significant (P=0.014). Conclusion: Medical students majoring in oncology have a positive attitude towards doctor-patient communication and a high demand for DPC, but the participation rate still needs to be improved. It is suggested to build teaching cases closely related to medical students majoring in oncology to better integrate teaching and clinical activities, theory and practice, and to implement stratified teaching according to the degree types and needs of medical students.
