医学科普训练对培养全科住培生沟通能力的效果研究

  • 摘要:目的 研究医学科普训练对提高全科住培生沟通能力的效果,为全科住培教育中的医患沟通培训体系提供理论依据。方法 选取2022年11月—2023年7月在湖北省第三人民医院进行住院医生规范化培训的全科住培生36名作为观察组,对其实施统一医学科普训练;另选取同期36名全科住培生作为对照组,采用传统教学法进行医患沟通能力培养。采用医患沟通技能测量工具医患沟通技能评价(Set Elicit Give Understand End,SEGUE)量表对两组全科住培生的临床沟通能力进行评分,对比两组全科住培生各分项得分、总得分及得分率情况;并收集两组住培生对医患沟通能力训练效果的评价进行比较。结果 观察组全科住培生的准备阶段、收集信息阶段、信息给予阶段、理解患者阶段、结束问诊等各分项相关评分(3.81±1.02)、(6.40±2.12)、(3.21±0.84)、(3.60±0.60)、(1.04±0.31)分均高于对照组(2.30±1.21)、(5.11±2.14)、(2.12±1.10)、(2.10±1.10)、(0.50±0.20)分,差异有统计学意义(P<0.05)。SEGUE量表总分观察组(17.44±3.72)高于对照组(12.02±4.31),其总得分率为69.60%,明显高于对照组48.00%。两组全科住培生对医患沟通能力训练评价的比较存在显著差异,对照组很满意、较满意、较不满意、很不满意率分别为8.33%、50.00%、44.44%、16.67%,观察组依次为22.22%、63.89%、22.22%、2.78%。 结论 加强医学科普训练可显著提高全科住培生的医患沟通技能,尤其在对患者的理解度及结束问诊阶段上有明显提升,临床实践价值较高。医学科普训练能大大提高全科住培生对医患沟通能力训练评价的满意度,值得在全科住培教育中广泛应用。

    Abstract: Objective:To study the effect of medical popular science training on improving the communication ability of general resident students, and to provide theoretical basis for the doctor-patient communication training system in general resident education. Methods:A total of 36 general practitioners who underwent standardized training for resident doctors at the Third Peoples Hospital of Hubei Province from November 2022 to July 2023 were selected as the observation group for psychological stress, and unified medical science popularization training was conducted on them; Another 36 general practitioners from the same period were selected as the control group, and traditional teaching methods were used to cultivate doctor-patient communication skills. Using the SEGUE Doctor Patient Communication Skills Evaluation Scale (SEGUE), the clinical communication skills of two groups of general practitioners were evaluated, and the scores of each sub item, total score, and scoring rate of the two groups of general practitioners were compared; Collect and compare the evaluations of two groups of resident trainees on the training effectiveness of doctor-patient communication skills. Results:The preparation stage, information collection stage, information delivery stage, understanding patient stage, and ending consultation stage of the observation groups general residency trainees were all significantly higher than the control groups (2.30±1.21), (5.11±2.14), (2.12±1.10), (2.10±1.10), (3.21±0.84), (3.60±0.60), and (1.04±0.31) scores, with statistical significance (P<0.05). The total score of the SEGUE scale was higher in the observation group (17.44±3.72) than in the control group (12.02±4.31), with a total score rate of 69.60%, significantly higher than 48.00% in the control group. There is a significant difference in the evaluation of doctor-patient communication ability training between two groups of general practitioners. The control group had very satisfied, relatively satisfied, relatively dissatisfied, and very dissatisfied rates of 8.33%, 50.00%, 44.44%, and 16.67%, respectively, while the observation group had rates of 22.22%, 63.89%, 22.22%, and 2.78%, respectively. Conclusion:Strengthening medical science popularization training can significantly improve the doctor-patient communication skills of general resident Pearson, especially in the understanding of patients and the end of the consultation stage, which has high clinical practice value. Medical popular science training can greatly improve the satisfaction of general resident students on the evaluation of doctor-patient communication ability training, and it is worth applying widely in general resident education.
