Assessment of consistency in the risk of bias in medical randomized controlled trials in China

• 摘要：目的/意义 医学文献的爆炸式增长使得临床决策依赖高质量证据，如何确保证据的一致性成为亟待解决的问题。多种偏倚评估工具的不一致解读会降低元分析的可靠性，甚至误导临床决策。本文旨在为临床研究和元分析提供更科学统一的评估标准，促进医学证据质量的提高。方法/过程 本研究通过检索中国期刊全文数据库，筛选出390条重复的风险偏倚评估记录。对这些记录从整体结果一致性、专题和内容一致性以及指标一致性三个方面进行分析。结果 超过80%的组别对于同一随机对照试验的风险评估不一致。在主题内容上，中医学的研究一致性较低，且在评估治疗的有效性和安全性方面一致性较差。在指标方面，选择性报道、结果数据的完整性和其他偏倚分歧较大。

  Abstract: Objective/Significance The explosive growth of medical literature has made clinical decision-making reliant on high-quality evidence, and the consistency of this evidence has become an urgent issue to be addressed. Inconsistent interpretations of various bias assessment tools can lead to a reduction in the reliability of meta-analysis results and may mislead clinical decisions. This study aims to provide more scientific and unified evaluation standards for clinical research and meta-analysis, thus promoting the improvement of medical evidence quality. Methods/Process 390 duplicate risk bias assessment records were retrieved from the CNKI database. These records were analyzed from three perspectives: overall result consistency, thematic and content consistency, and indicator consistency. Results Over 80% of the groups exhibited inconsistent risk assessments for the same randomized controlled trial. In terms of thematic content, the consistency of research in Traditional Chinese Medicine was found to be relatively low, with poor consistency in assessing treatment efficacy and safety. Significant differences were found in the indicators of selective reporting, completeness of outcome data, and other biases.
