2013-2022年北京地区住院患者流动就诊情况分析

  • 摘要：目的:了解北京地区住院患者流动就诊的变化趋势与规律,为卫生健康管理决策提供参考依据。方法:采用描述性方法对2013-2022年北京市二级及以上医疗机构住院患者跨省流入和北京市内跨区流动情况进行分析,采用χ2检验方法比较2020-2022年与2013-2019年外地患者占比和跨区就诊率的差异。结果:2013-2022年北京地区二级及以上医疗机构住院患者共2872.5万人次,其中跨省来京患者690.1万人次(占24.0%),主要来自于北京周边的河北、内蒙古、山东、山西和黑龙江等地,上述五省患者占全部跨省流入患者的63.8%；2013-2022年北京本地住院患者共2182.3万人次,其中市内跨区就诊患者1036.0万人次(占47.5%),郊区县向中心城区流入现象较为明显。流入患者最多的区为西城区,其跨省和跨区流入患者占比分别为27.6%与25.2%。三级甲等医院是流入患者最主要的选择,其跨省和跨区流入患者占比分别为84.1%和73.8%。2020-2022年期间,北京地区跨省来京住院患者占比较2013-2019年显著下降,而北京市内跨区就诊比例较2013-2019年显著升高(P<0.05)。结论:北京市住院患者跨省流动和市内跨区流动性均较大,流入地区和流入医院具有高度集中性,建议进一步优化区域医疗资源配置,引导患者合理有序就医。

    Abstract: Objective: To analyze the changing trend and regularity of cross-regional medical treatment-seeking of the hospitalized patients in Beijing in order to provide a statistic basis for decision making on the health management. Methods: A descriptive method was used to analyze the inter-provincial mobility and inter-district mobility of the hospitalized patients from the secondary and tertiary medical institutions in Beijing from 2013 to 2022, and χ2 test was applied to compare the difference in the proportion of cross-regional patients between 2020-2022 and 2013-2019. Results: A total of 28.725 million patients were admitted to the secondary and tertiary medical institutions in Beijing during the period of 2013-2022, of which 6.91 million (24.0%) came from other provinces; The cross-provincial patients mainly came from Hebei, Inner Mongolia, Shandong, Shanxi, and Heilongjiang around Beijing, with 63.8% of all cross-provincial patients coming to Beijing from these five provinces. There were a total of 21.823 million local inpatients in Beijing from 2013 to 2022，of which 10.36 million (47.5%) were cross-district patients seeking medical treatment within the city. The mobility trends from suburban counties to central urban areas was presented in patients across districts.The district with the highest patients coming from other provinces and districts was Xicheng District, with 27.6% and 25.2% of all cross-provincial patients and cross-district patients, respectively.The Class-A tertiary hospital was the main choice of patients seeking medical treatment across regions, with 84.1% and 73.8% of all cross-provincial patients and cross-district patients, respectively. Compared with 2013-2019, the proportion of cross-provincial patients decreased significantly and the proportion of cross-district patients increased significantly in Beijing during the period of 2020-2022（P<0.05）.Conclusion: The proportions of non-local patients seeking hospitalization service in Beijing and local patients seeking hospitalization service in other districts in Beijing were high, and the targeting districts and targeting hospitals were highly concentrated.It is suggested that the authorities in health sector should further optimize the allocation of regional medical resources and guide patients to seek rationally medical treatment in an orderly manner.
