Association of Traditional Chinese Medicine Constitution Types with METS-VF and Metabolic Syndrome in Patients with Psoriasis Vulgaris

• 摘要：【】目的 探讨寻常型银屑病患者中医体质类型与内脏脂肪代谢评分(METS-VF)及代谢综合征(MS)的关系。 方法 纳入寻常型银屑病患者108例,收集一般资料、PASI评分、人体测量指标及FPG、TG、HDL-C等指标,进行中医体质辨识,计算METS-VF,并诊断MS。采用组间比较、Logistic回归、ROC曲线及正常BMI亚组分析。 结果 MS患病率为26.85%。体质以湿热质、血瘀质、痰湿质为主。湿热质MS患病率最高,为40.74%。METS-VF升高、湿热质及PASI≥10与MS独立相关。METS-VF识别MS的AUC为0.812,最佳截断值为7.56。正常BMI亚组中,METS-VF与代谢异常组分数量呈正相关。 结论 湿热质和痰湿质寻常型银屑病患者代谢异常负担较重。METS-VF可作为银屑病代谢风险辅助识别指标,体质辨识联合METS-VF有助于中西医结合分层管理。

  Abstract: Objective To investigate the association of traditional Chinese medicine constitution types with metabolic score for visceral fat(METS-VF)and metabolic syndrome(MS)in patients with psoriasis vulgaris. Methods A total of 108 patients with psoriasis vulgaris were enrolled. Clinical data, PASI score, anthropometric parameters, FPG, TG and HDL-C were collected. TCM constitution was identified, METS-VF was calculated, and MS was diagnosed. Group comparisons, logistic regression, ROC analysis and normal-BMI subgroup analysis were performed. Results The prevalence of MS was 26.85%. Damp-heat, blood-stasis and phlegm-dampness constitutions were predominant. Damp-heat constitution showed the highest MS prevalence(40.74%). Increased METS-VF, damp-heat constitution and PASI≥10 were independently associated with MS. The AUC of METS-VF for identifying MS was 0.812, with an optimal cutoff value of 7.56. In the normal-BMI subgroup, METS-VF was positively correlated with the number of metabolic abnormality components. Conclusion Patients with damp-heat and phlegm-dampness constitutions had a heavier metabolic burden. METS-VF may serve as an auxiliary indicator for metabolic risk assessment in psoriasis vulgaris, and its combination with TCM constitution identification may support integrated risk stratification.
