“Foundation, Integration, Application”: Theoretical Framework and Practice Pathway of a Three-Dimensional Competency System for Cultivating Composite Talents in TCM Acupuncture-Based Rehabilitation

• 摘要：本文立足政策、行业标准与现有文献，以“粗守形，上守神”中医思想和世界卫生组织康复胜任力架构（RCF）为理论基础，构建“守正-融通-致用”三维能力体系，旨在厘清针灸康复复合型人才培养对象并建立分层能力框架与培养路径；本文将培养对象划分为两类，A类以中医专业学生、中医执业医师为主体，培养目标为能够独立开展针灸操作的复合型医师，B类面向康复治疗学专业学生及在岗康复治疗师，定位为在医师指导下实施针灸辅助操作的人员，并针对两类人群分别设置配套课程、操作授权及不良事件处置规范，该分层培养模式有助于实现人才能力目标与操作权限的适配，相关设置仅为推荐方案，仍有待临床实践验证，可为中医药及医学院校针灸康复方向的教学改革提供借鉴。

  Abstract: Under the “Healthy China” strategy, acupuncture has become the most widely applied TCM-characteristic technique in rehabilitation medicine, yet the training system for composite talents remains underdeveloped: existing studies mostly enumerate training pathways within the broad category of “TCM rehabilitation” without focusing on acupuncture itself to build a systematic theoretical framework, and pay insufficient attention to institutional bottlenecks such as practice-scope alignment. Grounded in the classical acupuncture epistemology that “the inferior practitioner guards the form, while the superior practitioner guards the spirit,” and integrating modern competency theories such as the WHO Rehabilitation Competency Framework (RCF), this paper proposesa three-dimensional progressive competency system of “Foundation-Integration-Application”: Foundation emphasizes the grounding transmission of classical theory and manual technique; Integration emphasizes the integrative convergence of Chinese-Western rehabilitation concepts and intelligent technology; Application emphasizes the translational realization of clinical competence and practice-scope alignment. The three dimensions progress stepwise while remaining mutually interpenetrating, structurally isomorphic with acupuncture’s own clinical reasoning chain of “pattern differentiation-meridian tracing-qi arrival.” Based on this framework, practice pathways are constructed across curriculum restructuring, faculty development, platform upgrading, and evaluation reform, with concrete recommendations on regulating rehabilitation therapists’ scope of acupuncture practice, providing theoretical support and practical reference for teaching reform in acupuncture-rehabilitation talent cultivation.
