Treatment of Sleep Disorder Secondary to Tinnitus of Kidney Essence Deficiency Pattern

• 摘要：摘要：目的：观察针刺联合穴位注射治疗肾元亏损型耳鸣继发睡眠障碍的临床疗效。方法：纳入肾元亏损型耳鸣继发睡眠障碍患者60例，随机分为治疗组（针刺+穴位注射组）与对照组（安慰针刺+安慰穴位注射组），每组30例。两组均每周治疗3次，连续治疗4周。观察治疗前后两组患者的耳鸣评价量表（TEQ）、匹兹堡睡眠质量指数（PSQI）、耳鸣残疾评估量表（THI）、中医证候积分变化，比较两组的临床疗效。结果：治疗后，两组TEQ、PSQI、THI评分及中医证候积分均较治疗前显著下降（P<0.001），且治疗组评分下降幅度显著大于对照组（P<0.001）。耳鸣疗效方面，治疗组总有效率为96.55%，显著高于对照组的32.14%（?2=33.393，P<0.001）；睡眠障碍疗效方面治疗总有效率为96.55%，显著高于对照组的25.00%（?2=39.176，P<0.001）。结论 针刺联合穴位注射可显著改善肾元亏损型耳鸣继发睡眠障碍患者的耳鸣症状、睡眠质量，缓解耳鸣相关功能残疾，改善肾元亏损中医证候，疗效稳定且安全性良好，值得临床推广应用。

  Abstract: Abstract：Objective: To observe the clinical efficacy of acupuncture combined with acupoint injection in the treatment of sleep disorder secondary to tinnitus with kidney essence deficiency pattern.Methods: A total of 60 patients with sleep disorder secondary to kidney essence deficiency pattern tinnitus were enrolled and randomly divided into the treatment group (acupuncture plus acupoint injection group) and the control group (sham acupuncture plus sham acupoint injection group), with 30 cases in each group. Both groups received treatment 3 times a week for 4 consecutive weeks. The changes of Tinnitus Evaluation Questionnaire (TEQ) score, Pittsburgh Sleep Quality Index (PSQI) score, Tinnitus Handicap Inventory (THI) score, and Traditional Chinese Medicine (TCM) syndrome score of patients in the two groups before and after treatment were observed, and the clinical efficacy of the two groups was compared.Results: After treatment, the TEQ, PSQI, THI scores and TCM syndrome scores of the two groups were significantly decreased compared with those before treatment (P<0.001), and the magnitude of score reduction in the treatment group was significantly greater than that in the control group (P<0.001). In terms of tinnitus efficacy, the total effective rate of the treatment group was 96.55%, which was significantly higher than 32.14% of the control group (?2=33.393, P<0.001). In terms of sleep disorder efficacy, the total effective rate of the treatment group was 96.55%, which was significantly higher than 25.00% of the control group (?2=39.176, P<0.001).Conclusion: Acupuncture combined with acupoint injection can significantly improve tinnitus symptoms and sleep quality, alleviate tinnitus-related functional disability, and ameliorate the TCM syndrome of kidney essence deficiency in patients with sleep disorder secondary to kidney essence deficiency pattern tinnitus. This therapy has stable efficacy and good safety, and is worthy of clinical promotion and application.
