Clinical Efficacy of Traditional Chinese Medicine Fumigation and Acupoint Massage in Treatment of Preoperative Swelling of Calcaneal Fractures

• 摘要：目的探究中药熏蒸联合循经穴位按摩治疗跟骨骨折术前肿胀的临床疗效。方法选取2023年1月至2024年7月在浙江中医药大学附属第一医院住院治疗的60例跟骨骨折患者为研究对象，采用随机数字表法将患者分为对照组（甘露醇+抬高患肢+冰敷）和观察组（对照组基础上运用中药熏蒸联合循经穴位按摩），每组30例。观察并比较两组患者的皮纹出现时间及治疗前、治疗后第1、3、5天的视觉疼痛模拟评分法（visual analogue scale，VAS）评分、腘静脉血流速度及患侧与健侧的踝-跟-踝周长差值。结果观察组皮纹出现时间（4.20±0.48天）、VAS评分、腘静脉血流速度、踝-跟-踝周长差值明显优于对照组，差异有统计学意义（P＜0.05）。结论中药熏蒸联合循经穴位按摩能够缩短跟骨骨折患者术前皮纹出现的时间，减轻患者术前疼痛，加快腘静脉血流速度，在促进患者术前消肿方面有较高的临床应用价值。

  Abstract: ObjectiveTo evaluate the clinical efficacy of Traditional Chinese medicine fumigation combined with meridian acupoint massage in managing preoperative swelling in patients with calcaneal fractures. MethodsA total of 60 patients with calcaneal fractures hospitalized at Zhejiang Provincial Hospital of Traditional Chinese Medicine between January 2023 and July 2024 were enrolled as study subjects. Using a random number table, patients were allocated into a control group (mannitol + limb elevation + ice application, n=30) and an observation group (receiving additional Traditional Chinese Medicine fumigation combined with meridian acupoint massage, n=30). The skin wrinkle reappearance time, VAS scores, popliteal venous flow velocity, and ankle-heel-ankle circumference differences between affected and healthy sides before treatment and on days 1, 3, and 5 post-treatment were compared between the two groups. ResultsThe observation group demonstrated significantly better outcomes than the control group in skin wrinkle reappearance time (4.20±0.48 days), VAS scores, popliteal venous flow velocity, and ankle-heel-ankle circumference differences (all P<0.05). ConclusionThe combination of Traditional Chinese Medicine fumigation and meridian acupoint massage significantly shorten the time of skin wrinkle reappearance, alleviated pain, enhanced popliteal venous hemodynamics, and demonstrated considerable clinical value in promoting the preoperative edema resolution of patients.
