中医药疗法改善高尿酸血症疗效的网状Meta分析

摘要:摘要:目的 探讨多种中医药疗法改善高尿酸血症临床症状的疗效。方法 通过检索Cochrane Library、PubMed、中国知网、万方数据知识服务平台、维普中文科技期刊全文数据库、中国生物医学文献数据库中自2014年1月至 2024年7月发表的关于中医药疗法干预改善高尿酸血症的随机对照试验(RCT)。在两名研究者独立筛选文献、提取数据,并采用Cochrane系统评价手册5.3推荐的RCT偏倚风险评估工具评价纳入文献的偏倚风险之后,采用R 4.2.2软件和Stata16软件对不同干预措施下,高尿酸血症症状改善的总有效率、血尿酸水平和患者中医证候评分改善情况进行贝叶斯网状meta分析。结果?共纳入42项随机对照试验,包括3504例患者,共涉及9种中医药干预措施(中药汤剂、代茶饮、针刺、穴位埋线、中药灌肠、中医养生干预、运动疗法、穴位按摩、电针)。网状Meta分析结果显示,在改善血尿酸水平方面,穴位埋线成为最佳干预措施的可能性最大;在改善中医证候评分方面,中药汤剂成为最佳干预措施的可能性最大;在改善高尿酸血症临床症状总有效率方面,穴位埋线成为最佳干预措施的可能性最大。结论 穴位埋线在改善高尿酸血症临床症状总有效率和血尿酸水平方面有显著优势,中药汤剂在改善高尿酸血症患者中医证候评分方面有显著优势。但还需要通过开展更多高质量的研究进一步验证此结论。关键词:高尿酸血症;中医药疗法;网状Meta分析;随机对照试验;血尿酸
Abstract: Abstract Objective: Evaluate the efficacy of various traditional Chinese medicine therapies in improving the clinical symptoms of hyperuricemia. Methods: Cochrane Library, PubMed, CNKI, Wanfang Data Knowledge Service Platform, CQ VIP, China Biomedical Literature Database were searched to look for randomised controlled trials (RCT) of TCM interventions to improve symptoms of hyperuricemia published from January 2014 to July 2024. Two investigators independently screened the literature, extracted data, and evaluated the risk of bias of the included literature using the RCT bias risk assessment tool recommended by the Cochrane Manual of Systematic Review 5.3. Bayesian network meta-analysis was performed on the overall response rate for improvement in hyperuricemia symptoms, blood uric acid levels, and improvement in medical syndrome scores in patients under different interventions by using R 4.2.2 software and Stata16 software. Results: A total of 42 randomized controlled trials, including 3504 patients, involving a total of 9 TCM interventions were included (decoction, medicinal tea, acupuncture, catgut embedding, enema, life nurturing, kinesiatrics, massage, electroacupuncture). The results of the network meta-analysis showed that catgut embedding was the most likely to be the best intervention in improving blood uric acid levels; In terms of improving TCM syndrome scores, Chinese herbal decoctions are the most likely to be the best intervention; In terms of overall effectiveness in improving clinical symptoms of hyperuricemia, catgut embedding is the most likely to be the best intervention. Conclusion: Catgut embedding has significant advantages in improving the total efficiency and blood uric acid level of clinical symptoms of hyperuricemia and Chinese medicine decoction has significant advantages in improving the medical syndrome score of hyperuricemia patients. However, more high-quality studies are needed to further validate this conclusion.
