Meta-Analysis of the Efficacy and Safety of Traditional Chinese Medicine or Integrated Traditional Chinese and Western Medicine for Endometriosis

• 摘要：目的：系统评价中药或中西药结合治疗方案对比单纯西药治疗子宫内膜异位症（Endometriosis，EMS）的有效性与安全性，为临床实践提供循证依据。方法：计算机检索中国知网、PubMed、Web of Science 等数据库，搜集自建库至2025年9月发表的相关随机对照试验（RCT）。依据PIOS原则制定纳入与排除标准，由两名研究者独立筛选文献、提取资料并评估偏倚风险。采用RevMan5.4软件进行Meta分析，二分类变量以相对危险度（RR）表示，连续变量以标准化均数差（SMD）表示，并计算95%置信区间（CI）。结果：共纳入21项RCT，合计1584例患者。Meta分析结果显示：在总有效率方面，中药或中西药结合组优于单纯西药组（RR=1.12,95%CI1.04,1.22,P=0.005）；在不良反应发生率方面，实验组显著低于对照组（RR=0.17,95%CI0.12,0.26,P<0.00001）。而在血清CA125水平、E2水平及复发率方面，两组差异无统计学意义（P>0.05）。结论：本Meta分析表明，中药或中西药结合治疗EMS在提高总有效率和降低不良反应发生率方面优于单纯西药治疗，安全性良好。然而，在改善血清CA125、E2水平及降低复发率方面，未显示出显著优势。未来需更多高质量研究进一步验证其疗效并探索作用机制。

  Abstract: Aim: To systematically evaluate the efficacy and safety of Traditional Chinese Medicine (TCM) or integrated TCM and Western medicine compared with Western medicine alone in the treatment of endometriosis (EMS), and to provide evidence-based support for clinical practice. Methods: Randomized controlled trials (RCTs) published from database inception to September 2025 were retrieved through electronic searches of databases including CNKI, PubMed, and Web of Science. Inclusion and exclusion criteria were formulated according to the PICOS principle. Two researchers independently screened the literature, extracted data, and assessed the risk of bias. Meta-analysis was performed using RevMan 5.4 software. Dichotomous variables were expressed as relative risk (RR), continuous variables were expressed as standardized mean difference (SMD), and both were presented with 95% confidence intervals (CI). Results: A total of 21 RCTs involving 1584 patients were included. The meta-analysis results showed that regarding the overall response rate, the TCM or integrated medicine group was superior to the Western medicine alone group (RR=1.12, 95% CI 1.04,1.22, P=0.005). Regarding the incidence of adverse reactions, the experimental group was significantly lower than the control group (RR=0.17, 95% CI 0.12, 0.26, P<0.00001). However, no statistically significant differences were found between the two groups in terms of serum CA125 levels, E2 levels, or recurrence rate (P>0.05). Conclusion: This meta-analysis indicates that TCM or integrated TCM and Western medicine is superior to Western medicine alone in improving the overall response rate and reducing the incidence of adverse reactions in the treatment of endometriosis, demonstrating a favorable safety profile. However, no significant advantages were shown in improving serum CA125 and E2 levels or reducing the recurrence rate. Future high-quality studies are warranted to further validate its efficacy and explore the underlying mechanisms.
