多靶点交替tDCS联合任务导向性训练对运动发育迟缓儿童运动功能及表面肌电活动的影响

Effects of Multi-Target Alternating Transcranial Direct Current Stimulation Combined with Task-Oriented Training on Motor Function and Surface Electromyographic Activity in Children with Motor Developmental Delay

  • 摘要:目的 探究多靶点交替经颅直流电刺激(transcranial direct current stimulation,tDCS)联合任务导向性训练对运动发育迟缓儿童运动功能及表面肌电活动的影响。方法 选取2023年1月—2024年2月赣州市妇幼保健院收治的80例运动发育迟缓儿童,根据随机数字表法分为对照组与观察组,各40例。对照组行单纯任务导向性训练,观察组采用多靶点交替tDCS联合任务导向性训练。比较两组临床总有效率,以及治疗前后Peabody运动发育量表-2(Peabody developmental motor scale, PDMS-2)、Berg平衡量表(Berg balance scale, BBS)评分及双侧肢体表面肌电均方根值(root mean square, RMS)差值。结果 观察组治疗总有效率显著高于对照组(85.00% vs 62.50%,P<0.05)。治疗后,观察组PDMS-2中粗大运动发育商较对照组提升3.27分、精细运动发育商提升4.76分、总体运动发育商提升3.68分,BBS评分提升2.44分,差异均有统计学意义(P<0.05)。治疗后,观察组双侧上肢肱二头肌及肱三头肌的表面肌电RMS差值分别较对照组降低1.57μV和2.60μV,双侧下肢腘绳肌与股四头肌的表面肌电RMS差值分别降低0.83μV和0.68μV,差异均有统计学意义(P<0.05)。结论 多靶点交替tDCS联合任务导向性训练能有效改善运动发育迟缓儿童的运动功能、平衡能力与表面肌电特征,并提升其双侧肢体肌肉活动对称性。

    Abstract: Objective To explore the effects of multi-target alternating transcranial direct current stimulation (tDCS) combined with task-oriented training on motor function and surface electromyographic (sEMG) activity in children with motor developmental delay. Methods A total of 80 children with motor developmental delay admitted to Ganzhou Maternal and Child Health Hospital from January 2023 and February 2024 were selected and divided into a control group and an observation group using a random number table, with 40 cases in each group. The control group received task-oriented training alone, while the observation group received multi-target alternating tDCS combined with task-oriented training. The total clinical effective rate, scores on the Peabody developmental motor scales-2 (PDMS-2), Berg balance scale (BBS) scores, and the difference in root mean square (RMS) values of bilateral limb sEMG signals before and after treatment were compared between the two groups. Results The total effective rate in the observation group was significantly higher than that in the control group (85.00% vs 62.50%, P<0.05). After treatment, compared with the control group, the observation group showed increases of 3.27 points in the gross motor quotient, 4.76 points in the fine motor quotient, and 3.68 points in the total motor quotient of the PDMS-2, as well as a 2.44-point increase in the BBS score, with all differences being statistically significant (P<0.05). Additionally, the RMS differences of surface electromyography in the bilateral biceps brachii and triceps brachii decreased by 1.57 μV and 2.60 μV, respectively, while those in the bilateral hamstrings and quadriceps decreased by 0.83 μV and 0.68 μV, respectively, with all differences being statistically significant (P<0.05). Conclusion Multi-target alternating tDCS combined with task-oriented training can effectively improve motor function, balance ability, and surface electromyographic features in children with motor developmental delay, and enhance the symmetry of bilateral limb muscle activity.
