    Abstract: 糖尿病心肌病(Diabetic cardiomyopathy,DCM)的定义是:在没有其他心脏危险因素(例如冠状动脉疾病、高血压或者糖尿病患者具有瓣膜病)的情况下,糖尿病患者存在心肌结构和表现异常,最终发展为心力衰竭1,DCM是糖尿病患者常见的心血管并发症之一,其中心血管疾病也是糖尿病患者死亡的主要原因,糖尿病患者常发生的是射血分数保留型心力衰竭 (Heart failure with preserved ejection fraction,HFpEF),可能占所有糖尿病患者心衰发生的 50%2。对于DCM的发生机制尚未完全明确,目前热门研究的机制中包括糖代谢异常及胰岛素抵抗、脂代谢、钙调节失衡以及氧化应激等3,氧化应激和线粒体功能障碍被认为是DCM发生的关键组成部分。核呼吸因子1(Nuclear respiratory factor 1 ,Nrf1)和线粒体转录因子A(Mitochondrial transcription factor A,TFAM)对线粒体功能稳定性起着一定作用,目前,对于Nrf1-TFAM通路介导的线粒体DNA的稳定性在DCM中的作用机制尚未完全清楚,本文就Nrf1与TFAM介导的线粒体DNA(Mitochondrial DNA,mtDNA)稳定性在DCM中的作用机制以及研究前景予以综述,以期为未来糖尿病心肌病的诊治上提供新思路。
