Abstract: Objective: To explore the clinical efficacy, safety, and acquired resistance characteristics of sacituzumab govitecan in FRα-positive, long-course, multi-line refractory platinum-resistant high-grade serous ovarian cancer. Methods: A retrospective analysis was conducted on a patient with high-grade serous ovarian cancer who had recurrent disease after multiple lines of anti-tumor therapy, was platinum-resistant, and exhibited FRα-positive expression. The patient received monotherapy with sacituzumab govitecan, followed by regular follow-up, dynamic monitoring of tumor markers CA125 and HE4, and imaging changes to evaluate efficacy and adverse reactions. Results: The patient initially showed significant benefits from sacituzumab govitecan treatment, with notable decreases in CA125 and HE4 levels, and imaging indicated stable tumor lesions, effectively controlling the disease. The drug-related adverse reactions were mild and tolerable. However, in the later stages of treatment, the patient developed acquired resistance, with persistent increases in CA125 and HE4 levels. Conclusion: Sacituzumab govitecan can provide short-term benefits for FRα-positive, multi-line refractory platinum-resistant high-grade serous ovarian cancer patients, with favorable safety. However, resistance progression is prone to occur in the later clinical stages, necessitating further clarification of resistance mechanisms and exploration of combination treatment strategies to delay resistance, offering clinical reference for the precise treatment of this refractory ovarian cancer.
