Abstract: Autoimmune encephalitis (AE) comprises a group of immune-mediated inflammatory disorders of the central nervous system. Its clinical manifestations typically include cognitive impairment, psychiatric and behavioral abnormalities, seizures, movement disorders, and sleep disturbances. Cognitive dysfunction is among the most common features across AE subtypes and may appear as an early symptom or persist as a long-term sequela. The prevalence and affected cognitive domains vary according to disease subtype, age of onset, disease severity, and the timing of treatment initiation. This review summarizes recent advances in the clinical characteristics, underlying mechanisms, and therapeutic approaches related to cognitive impairment in AE.
