Abstract: Objective:To explore the medication patterns of traditional Chinese medicine (TCM) in the treatment of liver cancer with ascites. Methods: A systematic retrieval was conducted in databases such as China National Knowledge Infrastructure (CNKI), Wanfang, and VIP. All clinical research literature on TCM treatment for liver cancer with ascites published between January 1, 2005, and August 31, 2025, was retrieved. Literature was screened according to inclusion criteria, an Excel database was constructed, data cleaning and TCM frequency statistics were performed using RStudio, and data analysis was conducted with SPSS Modeler 18.0 and SPSS Statistics 31.0. Results: A total of 22 articles were included, yielding 22 TCM prescriptions involving 90 types of herbs. The most frequently used TCM categories were tonifying agents, diuretic and dampness-resolving agents, and exterior-releasing agents. The primary properties of the herbs were cold, warm, and neutral, while their primary tastes were sweet, bitter, and pungent. The main meridians targeted were the Liver, Spleen, Lung, Stomach, and Kidney. Twelve high-frequency herbs were identified. The top three most frequently used herb pairs were Astragalus and Atractylodes, Bupleurum and Atractylodes, and Poria and Atractylodes. Commonly used herb combinations included Astragalus-Bupleurum-Atractylodes, Alisma-Bupleurum-Atractylodes, and Poria-Bupleurum-Atractylodes. Cluster analysis categorized the prescriptions into three major types: spleen-stomach harmonizing, liver-soothing, spleen-strengthening, and water-promoting, and water-draining, and spleen-nourishing and water-moving. Conclusion: The prescription principles of TCM for liver cancer with ascites are primarily based on tonification and elimination methods, emphasizing qi-moving, water-promoting, and edema-reducing effects, while also focusing on balancing yin and yang and protecting the spleen and stomach.
