Abstract: Objective To study social alienation profiles and frailty in elderly stroke patients.?Methods?A total of 312 elderly ischemic stroke patients were enrolled via convenience sampling between January 2025 to August 2025. Data were collected using the General Alienation Scale, Fried Frailty Phenotype, Fugl-Meyer Assessment, and CES-D. Latent profile analysis identified social alienation subgroups. Their association with frailty was analyzed with χ2 tests and binary logistic regression. Results Three profiles were identified: High (28.52%), Moderate (35.89%), and Low Alienation (35.57%). Both the moderate (OR=2.279, 95%CI:1.227-4.232) and high alienation groups (OR=3.379, 95%CI:1.560-7.317) were significant risk factors for frailty.? Conclusion?Moderate and high social alienation significantly increase frailty risk in elderly stroke patients. Interventions should be tailored according to alienation profile.
