Basic Research Progress on Rhubarb and Rhubarb-Based Formulae in the Intervention of Stroke

    Abstract: Secondary brain injury after stroke involves multiple interacting pathways. Within the gut–brain axis framework, the traditional Tongfu concept reflected in rhubarb and its formulae aligns with modern multi-target strategies. Studies show that their bioactive components suppress neuroinflammation, pyroptosis, ferroptosis, and oxidative stress. These actions synergistically stabilize blood–brain barrier tight junctions, remodel gut microbiota, and repair the intestinal barrier, thereby reducing gut-derived inflammation and regulating the interconnected network of inflammation, oxidative stress, and barrier impairment. From a multi-system perspective, this review integrates key mechanisms of rhubarb and its formulae in mitigating post-stroke secondary injury, offering a biological interpretation of Tongfu and a basis for further research on rhubarb-derived agents.
