Abstract: Abstract:Persistent Postural-Perceptual Dizziness (PPPD) is a chronic vestibular disorder characterized by persistent non-spinning dizziness and postural unsteadiness, often with a prolonged clinical course. In Traditional Chinese Medicine (TCM), it falls under the category of vertigo (眩晕) and presents with complex pathogenesis, leading to diverse traditional treatment approaches. Based on the theory of Turbid Pathogen Harms the Clear (浊邪害清), Professor Wu Dahua proposes that the etiology of PPPD is a deficiency-rooted excess pattern. This signifies a deficiency of kidney essence as the root cause, while turbid pathogens such as phlegm-turbidity and stasis-toxin clouding the clear orifices constitute the secondary manifestations. The treatment strategy, therefore, emphasizes strengthening the root by boosting the kidney and replenishing essence, while simultaneously addressing the manifestations by eliminating phlegm, resolving stasis, and unblocking the orifices and collaterals—thereby tackling both aspects concurrently. This article summarizes Professor Wus clinical experience in diagnosing and treating PPPD guided by the Turbid Pathogen Harms the Clear theory, aiming to provide a reference for TCM in the prevention and treatment of this condition.
