A Brief Discussion on the Treatment Experience of Anal Fistula in Crohns Disease Based on the Theory of Root and Manifestation, Deficiency and Excess

  Abstract: Crohns disease is a chronic and non-specific intestinal inflammatory disorder. Up to now, its exact etiology remains incompletely understood, and the pathogenesis is rather intricate. Clinically, it exhibits a wide spectrum of diverse manifestations. For some patients, at the early stage of the disease, the initial symptoms often mainly present as local perianal symptoms, among which perianal fistulas are particularly prevalent.In the theoretical framework of traditional Chinese medicine, the anal fistulas induced by Crohns disease are categorized into the realm of root deficiency and manifestation excess. To be more specific, the deficiency of both the spleen and the kidney serves as the fundamental internal cause. The spleen, being responsible for the transportation and transformation of nutrients, and the kidney, which is crucial for the bodys vital essence and energy, when deficient, weaken the bodys overall constitution. On the other hand, the downward deposition of dampness-toxicity evil qi in the perianal area is the external manifestation. Dampness and toxicity can obstruct the normal flow of qi and blood, leading to local inflammation and the formation of fistulas.Given the highly variable nature of Crohns disease, the treatment strategy emphasizes the integration of holistic and local therapies. A comprehensive assessment of the patients disease state, including the severity of the intestinal inflammation, the condition of the perianal fistula, and the patients overall physical constitution, is essential for formulating a personalized treatment plan. This may involve a combination of traditional Chinese medicine treatments such as herbal medicine to tonify the spleen and kidney and clear away dampness and toxicity, as well as surgical interventions for the management of the anal fistula.In conclusion, understanding the concept of root and manifestation, deficiency and excess is of great significance in the treatment of anal fistulas in Crohns disease. It provides a unique perspective for guiding clinical practice and developing more effective treatment approaches to improve the prognosis and quality of life of patients.
