Abstract: Objective: To analyze the clinical efficacy of one-time massage bone-setting manipulation in the treatment of lumbar disc herniation (LDH) based on the changes of surface electromyogram signals. Methods: 70 patients with LDH admitted to the Tuina Department of Zhejiang Provincial Hospital of Traditional Chinese Medicine between July 2023 and July 2024 were selected and randomly divided into an observation group (n=35) and a control group (n=35). The observation group received one-time massage bone-setting manipulation, while the control group received conventional massage techniques combined with the oblique-pulling manipulation. Both groups were treated for two weeks. The clinical efficacy of the two groups was evaluated. Comparisons were made before and after treatment regarding the traditional Chinese medicine (TCM) syndrome scores, Visual Analog Scale (VAS) scores, Japanese Orthopedic Association (JOA) low back pain scores, surface electromyogram signals root mean square (RMS) and median frequency (MF) of the erector spinae muscle, and the total incidence of adverse reactions. Results: The clinical efficacy of the observation group was superior to that of the control group (P<0.05). After treatment, both groups showed significant improvement in VAS scores, JOA scores, RMS values, MF values, and TCM symptom scores compared to before (P<0.05). In the intergroup comparison, the observation group demonstrated more significant improvements in these scores than the control group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion: One-time massage bone-setting manipulation for LDH can effectively alleviate pain, improve lumbar function, and enhance the quality of life.
