Clinical Efficacy of One-Time Massage Bone-Setting Manipulation in the Treatment of Lumbar Disc Herniation Based on Surface Electromyogram Signal Analysis

  • 摘要:目的:基于表面肌电信号的变化分析一次推拿正骨手法治疗腰椎间盘突出症(lumbar disc herniation,LDH)的临床疗效。方法:选取浙江省中医院推拿科2023年7月至2024年7月收治的LDH患者70例,随机分为观察组35例和对照组35例,观察组采用一次推拿正骨手法治疗,对照组采用常规推拿手法结合斜扳法,均治疗2周。评价两组的临床疗效。比较两组治疗前后中医症候评分、视觉模拟评分法(visual analog scale,VAS)、日本骨科协会(Japanese orthopedic association,JOA)腰痛评分、表面肌电信号竖脊肌均方根肌电值(root mean square,RMS)和中位频率(median frequence,MF)及不良总反应发生率。结果:观察组临床疗效优于对照组(P<0.05)。治疗后,两组的VAS评分、JOA评分、RMS值 MF值及中医症候评分均较前改善(P<0.05);且两组间比较,观察组上述评分优于对照组(P<0.05)。两组不良反应总发生率无统计学差异(P>0.05)。结论:一次推拿正骨法治疗LDH可有效改善患者疼痛及腰椎功能,提高生活质量。

    Abstract: Objective: To analyze the clinical efficacy of one-time massage bone-setting manipulation in the treatment of lumbar disc herniation (LDH) based on the changes of surface electromyogram signals. Methods: 70 patients with LDH admitted to the Tuina Department of Zhejiang Provincial Hospital of Traditional Chinese Medicine between July 2023 and July 2024 were selected and randomly divided into an observation group (n=35) and a control group (n=35). The observation group received one-time massage bone-setting manipulation, while the control group received conventional massage techniques combined with the oblique-pulling manipulation. Both groups were treated for two weeks. The clinical efficacy of the two groups was evaluated. Comparisons were made before and after treatment regarding the traditional Chinese medicine (TCM) syndrome scores, Visual Analog Scale (VAS) scores, Japanese Orthopedic Association (JOA) low back pain scores, surface electromyogram signals root mean square (RMS) and median frequency (MF) of the erector spinae muscle, and the total incidence of adverse reactions. Results: The clinical efficacy of the observation group was superior to that of the control group (P<0.05). After treatment, both groups showed significant improvement in VAS scores, JOA scores, RMS values, MF values, and TCM symptom scores compared to before (P<0.05). In the intergroup comparison, the observation group demonstrated more significant improvements in these scores than the control group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P>0.05). Conclusion: One-time massage bone-setting manipulation for LDH can effectively alleviate pain, improve lumbar function, and enhance the quality of life.
