A Brief Exploration of the Method of Seeds in Xu Dachuns The Essentials of Gynecology

• 摘要：目的：徐大椿所撰《女科指要》乃清代妇科典籍之佳构，探求“种子门”篇对不孕症理、法、方、药系统而细致的阐释。方法：以文献考据与归纳总结为主，结合脉证辨析推衍病机方药。结果：其论指出，不孕之因不仅关乎女子，亦与男子相关；而于女子而言，多由精血亏虚，冲任失调所致。常见脉象多呈微弱涩滞，遂确立“养血调经”为基本治则。在方药运用上，以四物汤为核心方剂，随证加减以应气滞、血瘀、水饮等不同病机，并善用剂型之变以契合病势，力求疗效之臻。尤对于久治未孕者，常佐以血肉有情之品，以厚填精血，展现出其独具匠心的诊疗思路。结论：书中体现的理论与实践不仅折射出清代妇科医家之深邃见解，亦为后世学人提供了宝贵的参考与借鉴。

  Abstract: Purpose: Nu Ke Zhi Yao (The Essentials of Gynecology), compiled by Xu Dachun, is an excellent classic of gynecology in the Qing Dynasty. This study explores the systematic and detailed explanations of the pathogenesis, treatment principles, prescriptions, and medicines for infertility in the chapter Zhongzi Men (Section on Conception).Methods: The research mainly adopts the methods of literature textual research and inductive summary, combined with the analysis of pulse conditions and symptoms to deduce the pathogenesis, prescriptions, and medicines.Results: The book points out that the causes of infertility are not only related to women but also to men. For women, infertility is mostly caused by deficiency of essence and blood, as well as disharmony of the Chong and Ren Meridians. The common pulse conditions are mostly weak, thready, and hesitant. Therefore, nourishing blood and regulating menstruation is established as the basic treatment principle. In the application of prescriptions and medicines, Siwu Decoction (a classic TCM prescription for nourishing blood) is used as the core formula, with modifications and additions according to different pathogenesis such as qi stagnation, blood stasis, and fluid retention. It also makes good use of changes in dosage forms to adapt to the progression of the disease and strive for the best curative effect. Especially for cases of long-term infertility, substances with rich nutritive properties (referring to animal-based TCM ingredients that replenish essence and blood) are often added to enrich essence and blood, demonstrating its unique and ingenious diagnosis and treatment ideas.Conclusion: The theory and practice embodied in the book not only reflect the profound opinions of gynecologists in the Qing Dynasty, but also provide valuable reference for later generations of scholars.
