基于国家专利复方治疗桥本甲状腺炎的中药配伍规律研究

摘要:目的 通过数据挖掘技术分析国家专利防治桥本甲状腺炎的用药规律,为临床诊疗和新药开发提供参考。方法 总结国家知识产权局专利数据库与中国知网专利数据库中防治桥本甲状腺炎的中药复方专利,搜索时间为自建库至2024年12月31日,借助Excel 2019、古今医案云平台等数据挖掘软件对专利复方中药进行使用频次、性味归经、关联规则、药物聚类、复杂网络等进行分析等。结果 通过筛选,符合纳入标准的专利复方共计57项,涉及215味中药,使用频次较高的前6味中药是黄芪、柴胡、夏枯草、白芍、甘草、茯苓。药性以为寒、平、温为主,药味以苦、甘、辛为主;归经以肝经最多,其次是脾经、肺经。关联性强的对药3组,角药1组,聚类分析共获得4个高频药物组合。结论 桥本甲状腺炎基本病机多以正气内虚为本,气滞、血瘀、痰凝搏结于瘿处为标,属本虚标实,虚实夹杂之证。临床多用补虚药黄芪、甘草、党参、白术等以固其本,疏肝理气药柴胡、香附等,化痰散结药夏枯草、浙贝母等,活血化瘀药当归、川芎、丹参等以治其标,标本同治,防微杜渐。
Abstract: Objective To analyze the medication pattern of national patents for the prevention and treatment of Hashimotos thyroiditis by means of data mining technology, so as to provide reference for clinical diagnosis and treatment and new drug development. Methods To summarize the Chinese medicine compound patents for the prevention and treatment of Hashimotos thyroiditis in the patent database of the State Intellectual Property Office and the patent database of China Knowledge Network, with the search time from the establishment of the database to December 31, 2024, with the help of Excel 2019, the ancient and modern medical case cloud platform, and other data mining software, we analyzed the frequency of use of the patented compound Chinese medicines, the sex and flavor attribution, the rules of association, the clustering of medicines, and the complex network, and so on. Results Through screening, a total of 57 patented compound prescriptions met the inclusion criteria, involving 215 Chinese medicines, and the top 6 Chinese medicines with higher frequency of use were Astragalus, Chaihu, Xiaku Cao, Paeonia lactiflora, Glycyrrhiza glabra, and Poria. The medicinal properties were mainly cold, calm and warm, and the flavors were mainly bitter, sweet and pungent; the categorization of meridians was most in the liver meridian, followed by the spleen meridian and lung meridian. There were three groups of strongly related pairs of drugs and one group of angular drugs, and a total of four high-frequency drug combinations were obtained by cluster analysis. Conclusion The basic pathogenesis of Hashimotos thyroiditis is mostly based on the internal deficiency of positive qi, and the stagnation of qi, blood stasis, and phlegm congealment at the galls as the symptoms, which is the evidence of the deficiency of the root and the mixture of deficiencies and realities. Clinical use of deficiency tonifying drugs such as astragalus, licorice,codonopsis, atractylodes, etc. to strengthen the root, liver-dispersing drugs such as Chai Hu, Xiang Fu, etc., phlegm-dispersing drugs such as Xia Ku Cao, Zhe Bei Mu, etc., blood stasis activating drugs such as Angelica sinensis, Rhizoma Ligusticum Chuanxiong, Salviae Miltiorrhizae, etc.,in order to treat the symptom.
