  • 摘要:中图分类号:R714:目的 分析孕早期血清学筛查指标妊娠相关血浆蛋白A(pregnancy associated plasma protein-A,PAPP-A)、游离β-人绒毛膜促性腺激素(free beta-human chorionic gonadotropin ,free-hCGβ)、孕期体重增长(Gestational weight gain,GWG)与胎儿生长受限(fetal growth restriction FGR )的关系,探索其对FGR的预测价值。方法 回顾性选取2021年1月1日至2024年1月于内蒙古自治区妇幼保健院规律孕检、孕早期于该院行血清学筛查、确诊并分娩的FGR患者作为病例组(n=426例),随机选择同期胎儿体重正常单胎妊娠孕妇作为对照组(n=520例)。采用SPSS24.0统计学软件分析两组孕妇基本资料、血清学筛查资料,并通过受试者工作特征(receiver operating characteristic ,ROC)曲线分析血清学筛查指标、GWG对FGR预测价值。结果(1)FGR组孕妇年龄31.49±2.29岁高于对照组30.89±2.20岁(P<0.05),FGR组GWG、PAPP-A、free-hCGβ低于对照组(P<0.05)(2)年龄、PAPP-A降低、free-hCGβ降低、GWG降低是FGR发生的独立危险因素(3)PAPP-A、free-hCGβ、年龄、GWG联合预测FGR的AUC为0.806(95%CI:0.778~0.834)。结论 FGR发生的影响因素包括:PAPP-A、free-hCGβ、年龄、GWG;PAPP-A、free-hCGβ、年龄、GWG联合预测FGR的价值较高。

    Abstract: Objective To analyze the relationship between first-trimester serological screening markers pregnancy-associated plasma protein-A(PAPP-A), free beta-human chorionic gonadotropin (free-hCGβ), gestational weight gain (GWG), and fetal growth restriction (FGR), and to explore their predictive value for FGR. Method A retrospective study was conducted on 426 FGR patients (case group) who underwent regular prenatal care, first-trimester serological screening, and delivered at Inner Mongolia Maternal and Child Health Care Hospital from January 1, 2021, to January 2024. A control group of 520 normal-weight singleton pregnancies from the same period was randomly selected. SPSS 24.0 was used to analyze maternal characteristics and serological data. Receiver operating characteristic (ROC) curves were generated to evaluate the predictive value of serological markers and GWG for FGR. Results(1)Maternal age in the FGR group (31.49 ± 2.29 years) was higher than in the control group (30.89 ± 2.20 years,P< 0.05). GWG, PAPP-A, and free-hCGβ levels were significantly lower in the FGR group (P< 0.05).(2)Advanced maternal age, decreased PAPP-A, reduced free-hCGβ, and insufficient GWG were identified as independent risk factors for FGR.(3)The combined predictive model (PAPP-A, free-hCGβ, maternal age, and GWG) achieved an AUC of 0.806 (95% CI: 0.778–0.834) for FGR prediction. ConclusionKey factors influencing FGR include PAPP-A, free-hCGβ, maternal age, and GWG. The combination of these markers demonstrates high predictive value for FGR.
