Study on physicians’ choice preference for internet diagnosis and treatment services based on discrete choice experiments

• 摘要：目的：研究医生互联网医院诊疗服务的选择偏好及其影响因素，了解医生参与互联网医院诊疗服务的偏好、意愿和需求。方法：应用离散选择实验方式，抽取北京市三所综合医院及三所专科医院共119位医生进行问卷调查，应用条件Logit回归分析其选择偏好。结果：医生支付意愿从高到低为：医生对线上诊疗绩效分成比例较线下的变化、每日工作时长、响应时间。不同年龄、职称、工作年限及科室的医生选择偏好存在差异。结论：医生更偏向于在线上诊疗绩效分成比例较线下增加20%、互联网诊疗服务占年度考核的权重占比增加10%、每日工作时长为线下减少1h，线上增加1h、响应时间为24小时以内的情形下选择互联网医院诊疗服务。可对医生采取经济激励和非经济激励相结合、合理分配工作量等激励机制，满足医生的偏好和需求，促进互联网医院高质量发展。

  Abstract: Objective: To study physicians’ preference for Internet hospital diagnosis and treatment services and its influencing factors, and to understand doctors preference, willingness and demand for participating in Internet hospital diagnosis and treatment services. Methods: A total of 119 physicians from three general hospitals and three specialized hospitals in Beijing were selected for questionnaire survey by applying discrete choice experiment, and their choice preferences were analyzed by applying conditional logit regression. Results: From high to low, physicians’ willingness to pay is as follows: the change of doctors’ share of online diagnosis and treatment performance compared with offline, daily working hours and response time. Sub-group analysis showed differences in physicians’ ch oice preferences in different ages, professional titles, working years and departments. Conclusions: Physicians are more inclined to choose Internet hospital diagnosis and treatment services when the proportion of online diagnosis and treatment performance is increased by 20% compared with offline, the proportion of online diagnosis and treatment services in the annual assessment is increased by 10%, the daily working hours are reduced by 1h offline, the online time is increased by 1h, and the response time is less than 24 hours. Incentive mechanisms such as a combination of economic and non-economic incentives and reasonable distribution of workload can be adopted for doctors to meet their preferences and needs and promote the high-quality development of Internet hospitals.
