Clinical Value of Dyclonine Hydrochloride Mucilage-assisted Colonoscopy: A Systematic Review and Meta-analysis

• 摘要：目的 系统评价盐酸达克罗宁胶浆辅助结肠镜检查的临床效果及安全性。方法 检索PubMed、Embase、Cochrane Library、Web of Science、CNKI、维普、万方及中国生物医学文献数据库，纳入2016年1月至2026年7月发表的相关RCT。由2名研究者独立筛选文献并提取资料，采用RevMan 5.4和Stata 17.0进行Meta分析。结果 纳入14项RCT，共4404例患者。与对照组相比，达克罗宁胶浆可提高祛泡效果（RR=1.25，95%CI：1.13～1.40，P＜0.001），缩短检查时间（MD=-3.61，95%CI：-5.35～-1.87，P＜0.001），降低腹胀发生风险（RR=0.19，95%CI：0.09～0.40，P＜0.001）；两组BBPS评分（MD=0.66，95%CI：-0.34～1.67，P＞0.05）及不良事件发生率（RR=0.88，95%CI：0.64～1.21，P＞0.05）差异均无统计学意义。敏感性分析结果较稳定，未发现明显发表偏倚。结论 盐酸达克罗宁胶浆辅助结肠镜检查可改善祛泡效果、缩短检查时间并减少腹胀发生，且安全性较好，但部分结局存在异质性，仍需要更多高质量研究验证。

  Abstract: Objective To systematically evaluate the clinical efficacy and safety of dicyclomine hydrochloride mucilage as an adjunct to colonoscopy.Methods PubMed, Embase, Cochrane Library, Web of Science, CNKI, VIP, Wanfang Data, and the Chinese Biomedical Literature Database were searched for relevant randomized controlled trials (RCTs) published from January 2016 to July 2026. Two researchers independently screened the literature and extracted data. Meta-analysis was performed using RevMan 5.4 and Stata 17.0. Results Fourteen RCTs involving 4,404 patients were included. Compared with the control group, dicyclomine hydrochloride mucilage significantly improved defoaming efficacy (RR=1.25, 95% CI: 1.13–1.40, P<0.001), shortened colonoscopy time (MD=-3.61, 95% CI: -5.35 to -1.87, P<0.001), and reduced the risk of abdominal distension (RR=0.19, 95% CI: 0.09–0.40, P<0.001). No significant differences were observed between the two groups in BBPS scores (MD=0.66, 95% CI: -0.34 to 1.67, P>0.05) or the incidence of adverse events (RR=0.88, 95% CI: 0.64–1.21, P>0.05). Sensitivity analyses yielded relatively stable results, and no significant publication bias was detected.Conclusion Dicyclomine hydrochloride mucilage as an adjunct to colonoscopy may improve defoaming efficacy, shorten examination time, and reduce the incidence of abdominal distension, with a favorable safety profile. However, given the heterogeneity observed in some outcomes, further high-quality studies are warranted to confirm these findings.
