Mechanisms and Clinical Research Progress of Zhenwu Decoction in the Treatment of Dilated Cardiomyopathy

• 摘要：扩张型心肌病（DCM）是以无异常负荷诱因下左心室腔扩大、收缩功能受损为核心特征，该病远期预后较差，单纯西医干预手段存在明显局限性。近年来，中医经典名方真武汤在DCM治疗领域效果突出。本文通过回顾以往文献分别从中医理论基础、药理作用机制及临床研究进展三个方面展开论述，探讨其通过调节细胞自噬、抑制心肌纤维化、改善钙稳态等多途径发挥治疗作用，并结合临床研究验证其疗效。旨在为中西医结合治疗DCM提供参考依据，促进中医药的创新发展与临床转化。

  Abstract: Dilated cardiomyopathy (DCM) is mainly characterized by left ventricular dilatation and impaired systolic function without abnormal loading factors, with poor long-term prognosis. Single Western medicine intervention has obvious limitations. In recent years, Zhenwu Decoction, a classic prescription of traditional Chinese medicine (TCM), has achieved remarkable curative effects in the treatment of DCM. By reviewing relevant literature, this paper elaborates on three aspects including TCM theoretical basis, pharmacological mechanisms and clinical research progress. It discusses its multi-target therapeutic effects through regulating cellular autophagy, inhibiting myocardial fibrosis, improving calcium homeostasis and other pathways, and verifies its clinical efficacy based on clinical studies. This paper aims to provide references for the integrated treatment of DCM with TCM and Western medicine, and boost the innovative development and clinical transformation of traditional Chinese medicine.
