  • 摘要:多囊卵巢综合征(polycystic ovary syndrome,PCOS)是育龄期女性常见的生殖内分泌疾病,其发病不仅与排卵障碍和高雄激素血症有关,还伴随肥胖、胰岛素抵抗、糖脂代谢异常及慢性低度炎症等代谢紊乱。近年来,国际专家提出将其重新命名为多内分泌代谢卵巢综合征(polyendocrine metabolic ovarian syndrome,PMOS),进一步强调代谢异常在疾病发生发展中的核心地位。胰高糖素样肽-1受体激动剂(glucagon-like peptide-1 receptor agonists,GLP-1RA)因具有减重、改善胰岛素抵抗和调节代谢等作用,逐渐成为PCOS治疗研究的热点。本文综述GLP-1RA在PCOS中的药理基础、作用机制及临床应用,重点总结其对体重、胰岛素抵抗、糖脂代谢、生殖内分泌及生育结局的影响,并分析联合治疗、安全性、局限性及新型肠促胰素药物前景,为PCOS个体化代谢管理提供参考。

    Abstract: Polycystic ovary syndrome (PCOS) is one of the most common endocrine disorders among women of reproductive age. In addition to reproductive dysfunction characterized by hyperandrogenism and ovulatory disorders, PCOS is frequently accompanied by obesity, insulin resistance, dyslipidemia, chronic low-grade inflammation, and an increased risk of long-term cardiometabolic complications. Recently, an international consensus proposed renaming PCOS as polyendocrine metabolic ovarian syndrome (PMOS), highlighting the central role of metabolic abnormalities in its pathogenesis and clinical management. Glucagon-like peptide-1 receptor agonists (GLP-1RAs), originally developed for the treatment of type2 diabetes and obesity, have attracted increasing attention in PCOS because of their beneficial effects on body weight reduction, insulin sensitivity, and metabolic regulation. This review summarizes recent advances in the pharmacological basis, mechanisms of action, and clinical applications of GLP-1RAs in PCOS. Current evidence indicates that GLP-1RAs can improve body weight, insulin resistance, glucose and lipid metabolism, and hyperandrogenism, and may contribute to improvements in menstrual regularity, ovulatory function, and reproductive outcomes. Emerging studies also suggest potential roles of GLP-1RAs in modulating inflammation, gut microbiota, oxidative stress, and ovarian microenvironment. This review covers the pharmacology, mechanisms, and clinical use of GLP-1RAs in PCOS, emphasizing impacts on weight, insulin resistance, metabolism, reproductive hormones, and fertility, while also addressing combination strategies, safety, current limitations, and emerging incretin therapies to support personalized PCOS management.
