Meta-synthesis of qualitative research on cognitive impairment experience in cancer patients

• 摘要：目的 系统评价癌症患者认知障碍体验的质性研究，为制订相关管理策略提供参考。方法 检索中英文数据库中关于癌症认知障碍体验的质性研究。采用澳大利亚乔安娜布里格斯研究所循证卫生保健中心质性研究质量评价标准进行文献质量评价，采用汇集性整合方法进行结果整合。结果 共纳入16篇文献，提炼出48个研究结果，归纳成15个新类别，最终形成5个整合结果：癌症患者认知障碍的形成原因；癌症患者认知障碍的相关体验；癌症患者认知障碍的负面影响；癌症患者认知障碍的支持需求；癌症患者认知障碍的应对策略。结论 癌症患者的认知障碍受多种因素影响，其症状体验对患者造成负面影响，患者渴望得到帮助，医护人员应予以个体化指导，实施全程管理，帮助其改善认知功能。

  Abstract: Objective To systematically review qualitative studies on the experiences of cognitive impairment among cancer patients and to inform the development of management strategies. Methods A systematic search was conducted in Chinese and English databases to identify qualitative studies on the experience of cognitive impairment in cancer patients. The Joanna Briggs Institute (JBI) Critical Appraisal Checklist for Qualitative Research was used to assess the quality of included studies. A pooled integration method was employed to synthesize the findings. Results A total of 16 studies were included. Forty-eight research findings were extracted, categorized into 15 new categories, and ultimately synthesized into five integrated results: causes of cognitive impairment in cancer patients; experiences related to cognitive impairment in cancer patients; negative impacts of cognitive impairment on cancer patients; support needs of cancer patients with cognitive impairment; and coping strategies for cognitive impairment among cancer patients. Conclusion Cognitive impairment in cancer patients is influenced by multiple factors, and its symptoms negatively impact patients. Patients express a strong desire for support. Healthcare providers should offer individualized guidance and implement comprehensive management throughout the care continuum to improve patients'' cognitive function.
