• 摘要：摘要：目的 构建基于 5A 模式的卒中后吞咽障碍（post stroke dysphagia,PSD）患者康复训练方案，为临床标准化康复干预提供循证工具。方法 组建团队经文献研究结合临床实践拟定方案初稿， 2 轮专家函询结合意见修订，再经预试验优化确立最终方案。结果 2 轮共纳入函询专家 20 名，专家积极系数、权威系数分别为 88.89%、93.75% 和 0.85、0.916，肯德尔协调系数从 0.363 升至 0.638（P<0.001），专家积极性、权威性及意见协调性良好；形成含评估、建议、达成共识、协助干预、随访支持 5 个阶段及全面评估、个性化训练等 32 项干预内容。结论 研究构建的基于 5A 模式的PSD康复训练方案科学、可行且具有针对性，可为进一步康复干预提供循证依据。

  Abstract: Abstract: Objective: To construct a rehabilitation training program for post-stroke dysphagia patients based on the 5A model, providing an evidence-based tool for standardized clinical rehabilitation intervention. Methods: A team was formed to draft the initial plan through literature review and clinical practice. Two rounds of expert consultation were conducted to revise the plan based on the feedback, and a pretest was carried out to optimize and finalize the program. Results: A total of 20 experts were included in the two rounds of consultation. The expert activity coefficient and authority coefficient were 88.89%, 93.75% and 0.85, 0.916 respectively. The Kendalls coefficient of concordance increased from 0.363 to 0.638 (P < 0.001), indicating good expert activity, authority and consensus. The final program consists of five stages: assessment, advice, consensus, assistance in intervention, and follow-up support, and 32 intervention contents including comprehensive assessment and individualized training. Conclusion: The rehabilitation training program for PSD based on the 5A model is scientific, feasible and targeted, providing an evidence-based basis for further rehabilitation intervention.
