摘要:溃疡性结肠炎(ulcerative colitis, UC)是一种慢性非特异性肠道炎症性疾病,发病率逐年上升。尽管结肠镜检查目前是UC的主要诊断和病情评估工具,但其侵入性、操作复杂性及高收费等因素在一定程度上限制了临床应用。近年来,C反应蛋白/白蛋白比值(C-reactive protein-albumin ratio, CAR)、中性粒细胞/白蛋白比值(neutrophil-albumin ratio, NAR)、血小板/白蛋白比值(platelet-albumin ratio, PAR)及C反应蛋白/间接胆红素比值(C-reactive protein-Indirect Bilirubin ratio, CRP/IBIL)等基于血液的复合生物标志物,凭借其便捷性和潜在临床价值,逐渐成为该领域研究的热点。本文对上述指标在UC活动度监测等方面的应用进展作一综述,重点分析其与临床评分系统、内镜评分系统之间的相关性,探讨其作为无创生物标志物诊断UC疾病的效能、优势和局限,旨在为UC的疾病管理探索新的思路与工具。
Abstract: Ulcerative colitis (UC) is a chronic non-specific inflammatory bowel disease, and its incidence is increasing year by year. Currently, although colonoscopy is the main method for the diagnosis and disease assessment of UC, its invasiveness, operational complexity and high cost have limited its clinical application to a certain extent. In recent years, blood-based composite biomarkers such as the C-reactive protein-albumin ratio (CAR), neutrophil-albumin ratio (NAR), platelet-albumin ratio (PAR), and C-reactive protein-Indirect Bilirubin ratio (CRP/IBIL) have gradually become research hotspots in this field due to their convenience and potential clinical value. This article systematically reviews the research progress of the above indicators in the monitoring of UC activity, etc. It focuses on analyzing their correlations with clinical scoring systems and endoscopic scoring systems, and deeply explores their diagnostic efficacy, advantages, and limitations as non-invasive biomarkers, aiming to provide new ideas and tools for the management of UC.
