Abstract: Sodium-glucose cotransporter 2 (SGLT2) inhibitors, a class of glucose-lowering agents that has drawn considerable attention in recent years, are supported by accumulating evidence of cardiovascular protective effects independent of their glucose-lowering action, particularly in inhibiting ventricular remodeling and regulating inflammation. Large-scale clinical trials and meta-analyses have shown that SGLT2 inhibitors confer consistent cardiovascular benefits regardless of the presence or absence of diabetes; because patients with diabetes have a higher baseline risk, they may show greater absolute risk reductions or greater improvements in surrogate endpoints. This article reviews the effects of SGLT2 inhibitors on ventricular remodeling and inflammation after acute myocardial infarction, compares the differences in efficacy between patients with and without diabetes and the possible mechanisms underlying these differences, and proposes a strategy for stratified clinical application based on existing evidence, with the aim of informing the precision use of this drug class.
