Abstract: Objective: To examine employment preferences of standardized residency training physicians in Shanghai and heterogeneity in primary care preferences. Methods: Online survey of 1,813 physicians using proportional stratified sampling. Mixed logit models applied for preference estimation, relative importance analysis, heterogeneity analysis, and scenario simulation. Results: Standardized training residents preferred tertiary hospitals most. Monthly income was the top determinant, followed by hospital level and work environment. Those with bachelors degrees lacking four certificates, working ≥50 hours weekly, and in general practice showed relatively positive preferences for primary care. Scenario simulations showed that improving work environment significantly boosted primary care willingness; compared with tertiary hospitals, primary facilities needed favorable benefits or career prospects plus better environment to attract these residents. Conclusions: Most residents preferred tertiary hospitals. Both economic and non-economic factors shaped their preferences, with heterogeneity across different characteristics. Optimizing policy tools is needed to enhance primary care willingness, alongside strengthening service capacity and building sustainable talent retention mechanisms.
