  Abstract: Insulin resistance plays a pivotal role in the pathogenesis of endometrial cancer. The triglyceride-glucose index and its variants, as practical surrogate markers for insulin resistance due to their simplicity and low cost, have shown potential in predicting endometrial cancer risk. This review focuses on the central role of insulin resistance in endometrial cancer, critically evaluates the evidence regarding the predictive and prognostic value of the TyG index, analyzes existing inconsistencies in conclusions, and discusses challenges in clinical translation along with future research directions, aiming to inform metabolism-related prevention and control strategies for endometrial cancer.
