Abstract: Objective: To investigate the clinical characteristics and influencing factors of postoperative pulmonary infection in patients with hypertensive intracerebral hemorrhage, and to identify independent risk factors for guiding preventive strategies. Methods: A total of 132 patients with hypertensive intracerebral hemorrhage who underwent surgical treatment at Mentougou Hospital, Beijing Tongren Hospital, Capital Medical University?from December 2022 to December 2024 were enrolled. Patients were categorized into the infection group (n=26) and non-infection group (n=106) according to the presence of postoperative pulmonary infection. Univariate analysis and multivariate logistic regression analysis were employed to identify independent risk factors. Results: Multivariate analysis showed that preoperative hypoalbuminemia (OR=2.892, 95%CI:1.67–4.78), underlying pulmonary disease (OR=4.925, 95%CI:2.32–9.80), preoperative Glasgow Coma Scale (GCS) score (OR=5.663, 95%CI:2.84–11.21), and duration of mechanical ventilation (OR=5.312, 95%CI:2.24–11.79) were independent risk factors for postoperative pulmonary infection (all P<0.05).Conclusion: Preoperative hypoalbuminemia, combined underlying pulmonary disease, preoperative GCS score ≤8 , and prolonged postoperative mechanical ventilation are high-risk factors for postoperative pulmonary infection in patients with hypertensive intracerebral hemorrhage, requiring targeted interventions.
