Abstract: Carotid ultrasound is a core non-invasive imaging tool with an irreplaceable role in assessing the risk of ischemic cardiovascular and cerebrovascular diseases. This article systematically reviews its application value, beginning with an explanation of the technical principles and evaluation system of carotid ultrasound, covering two-dimensional grayscale, Doppler, as well as emerging technologies such as three-dimensional ultrasound, contrast-enhanced ultrasound, and elastography. Furthermore, it highlights that carotid atherosclerosis is not only a direct cause of ischemic stroke but also a critical window and early warning signal for systemic atherosclerosis. By conducting an in-depth analysis of the close associations between carotid atherosclerosis and ischemic diseases in vascular beds such as the coronary, intracranial, lower limb, renal, and ophthalmic arteries, this article further clarifies the pivotal role of carotid ultrasound in assessing the risk of ischemic vascular diseases.
