摘要:线粒体遗传疾病主要是由线粒体DNA或核DNA的突变而引发,且主要是通过母系遗传。线粒体替代疗法(MRT)是近年来在生殖医学领域中备受关注的一项创新技术,该技术旨在将存在明确缺陷的线粒体与外源正常线粒体进行置换,以阻断线粒体遗传病的传播,同时也可在辅助生殖技术中提升高龄妇女卵母细胞质量,提高优质胚胎形成率。近年来,全球范围内关于MRT的研究和临床应用取得了显著进展,然而,在技术应用中也面临着重大挑战,其安全性和有效性问题亟待进一步验证,同时MRT的使用引发了广泛的伦理争议,这些因素均对MRT的推广和应用造成了影响。本文将对MRT在生殖医学领域应的最新研究进展、存在的技术挑战、相关伦理问题及对MRT未来的相关展望进行了综述。
Abstract: Mitochondrial genetic diseases are mainly caused by mutations in mitochondrial DNA or nuclear DNA, and are mainly inherited through the maternal line. Mitochondrial replacement therapy (MRT) is an innovative technology that has attracted much attention in the field of reproductive medicine in recent years. This technology aims to replace mitochondria with clear defects with exogenous normal mitochondria to block the transmission of mitochondrial genetic diseases. At the same time, it can also improve the quality of oocytes in elderly women and increase the formation rate of high-quality embryos in assisted reproductive technology. In recent years, significant progress has been made in global research and clinical application of MRT. However, major challenges are also faced in the application of this technology, and its safety and effectiveness need further verification. At the same time, the use of MRT has also sparked widespread ethical controversies, all of which have affected the promotion and application of MRT. This article reviews the latest research progress of MRT in the field of reproductive medicine, existing technical challenges, related ethical issues, and prospects for the future of MRT.
