• 摘要：膝骨关节炎是因膝关节退行性变而出现疼痛、肿胀、活动受限等为主要表现,属中医“痹证”范畴。膝痹是以外邪痹阻局部,致局部经脉不通,不通则痛。日久局部气机阻滞,导致痰浊、淤血等病理产物的产生,同时气血津液运行不畅,不能濡养出现不荣则痛。“络以通为用”是在叶天士络病学说的基础下提出对此类病证治疗的理论方法。笔者将两者结合起来,通过《临证指南医案》中的关于“络以通为用”的内容结合临床所使用经验方“自拟蠲痹散”对膝骨关节炎治疗的运用,以通络为主要治法,以期为临床治疗 KOA 提出新的思路和方法。

  Abstract: Knee osteoarthritis (KOA) is a degenerative joint disease characterized by pain, swelling, and limited mobility, which falls under the category of Bi syndrome in Traditional Chinese Medicine (TCM). Knee Bi syndrome arises from the obstruction of external pathogens in local regions, leading to meridian stagnation and subsequent pain. Prolonged stagnation of local qi movement results in pathological products such as phlegm-turbidity and blood stasis, while impaired circulation of qi, blood, and body fluids fails to nourish tissues, causing malnourishment-related pain. The principle The collaterals should be treated by promoting their patency (Luo Yi Tong Wei Yong), derived from Ye Tianshis collateral disease theory, provides a therapeutic framework for such conditions. This study integrates this theory with clinical practice by applying the empirical formula Self-formulated Juanbi San from Lin Zheng Zhi Nan Yi An (Clinical Guide Medical Case Records). Focusing on collateral-dredging as the primary treatment strategy, this approach aims to propose novel perspectives and methodologies for KOA clinical management.
