Research Progress on Desquamative interstitial pneumonitis

• 摘要：脱屑性间质性肺炎(desquamative interstitial pneumonitis,DIP)是一种特发性间质性肺炎,其主要病理特点是肺泡中广泛浸润了巨噬细胞,之后出现了间质炎症和纤维化的情况。目前尚不清楚DIP的病因,但与吸烟有密切关系。患者常表现为进行性加重的咳嗽和气促,高分辨率胸部CT是首选的影像学检查方法。尽管经支气管冷冻活检正在被研究作为一种替代方法,但外科肺活检仍然是诊断DIP的金标准。治疗DIP的标准方法是戒烟和类固醇治疗。一般情况下,患者的预后是良好的,大多数在戒烟和接受皮质类固醇治疗后病情会有所改善。

  Abstract: Desquamative interstitial pneumonitis (DIP) is an idiopathic interstitial pneumonia whose main pathological feature is the extensive infiltration of macrophages in the alveoli followed by interstitial inflammation and fibrosis. The etiology of DIP is unknown, but there is a strong association with smoking. Patients often present with progressively worsening cough and shortness of breath, and high-resolution chest CT is the imaging modality of choice. Surgical lung biopsy remains the gold standard for diagnosing DIP, although transbronchial frozen biopsy is being investigated as an alternative. The standard treatment for DIP is smoking cessation and steroid therapy. The prognosis for patients is generally favorable, with most improving after quitting smoking and receiving corticosteroid therapy.
