摘要:支气管肺发育不良(Bronchopulmonary Dysplasia, BPD)在临床多见于早产儿,其发病机制涉及炎症、氧化应激及肺组织异常修复,其中炎症反应是核心环节。近年研究发现,中性粒细胞胞外诱捕网(Neutrophil Extracellular Traps, NETs)作为一种独特的免疫防御机制,在BPD的肺部炎症与组织损伤中发挥重要作用。维生素D(Vitamin D, Vit D)具有调节免疫、抑制炎症、促进肺发育等多重生物学效应,可能通过调控NETs形成发挥肺保护作用。本文就NETs在BPD发病中的作用、Vit D对 NETs的调控机制及其在BPD防治中的研究进展进行综述,为深入理解BPD发病机制并探索防治新策略提供参考。
Abstract: Bronchopulmonary dysplasia (BPD) is a chronic lung disease predominantly affecting preterm infants, with a pathogenesis involving inflammation, oxidative stress, and aberrant tissue repair. Among these, inflammatory response is considered the central mechanism. Recent studies have revealed that neutrophil extracellular traps (NETs), as a unique immune defense mechanism, play a significant role in pulmonary inflammation and tissue injury associated with BPD. Vitamin D (Vit D) exerts multiple biological effects, including immune regulation, inflammation inhibition, and promotion of lung development, and its potential value in the prevention and treatment of BPD has garnered increasing attention. Emerging evidence suggests that Vit D may confer lung protection by modulating NETs formation through multiple signaling pathways involving reactive oxygen species scavenging, inhibition of PADI4 activity, and regulation of the inflammatory microenvironment. This review summarizes the role of NETs in the pathogenesis of BPD, the regulatory mechanisms of Vit D on NETs, and the research progress in the prevention and treatment of BPD, aiming to provide a reference for further understanding of BPD pathogenesis and for exploring novel preventive and therapeutic strategies.
