• 摘要：【摘要】目的：了解痛风患者服药的真实经历和感受，探讨影响服药依从性的促进及阻碍因素，为针对性制定服药管理措施提供参考依据。方法：采用目的抽样法，选取2025年6月-2025年9月贵州中医药大学第二附属医院风湿免疫科住院的16名痛风患者进行深入访谈，运用NVivo11.0软件分析访谈资料，以COM-B模型为理论基础提炼主题。结果：提炼出阻碍因素的7个亚主题：疾病和用药认知不足、药物不良反应、患者能力下降、社会支持不足、经济条件差与医疗资源获取费用高、用药信心不足和药物获益感低；促进因素4个亚主题；结构化用药习惯、社会支持充足、医患信任与治疗信心、疾病严重程度与可感知后果。结论：影响痛风患者服药依从性因素是多维度的，医务人员应根据患者服药依从性差具体原因提供个性化的健康教育。

  Abstract: 【Abstract】Objective :To understand the authentic medication experiences and perceptions of patients with gout, explore the facilitating factors and barriers affecting medication adherence, and provide a reference for the development of targeted medication management strategies. Methods: Using purposive sampling, 16 patients with gout hospitalized in the Department of Rheumatology and Immunology at the Second Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from June 2025 to September 2025 were selected for in-depth interviews. Interview data were analyzed using NVivo 11.0 software, and themes were extracted based on the COM-B model as the theoretical framework. Results: Seven sub-themes related to barriers were identified: insufficient disease and medication knowledge, adverse drug reactions, reduced patient capability, inadequate social support, poor economic conditions and high costs of accessing medical resources, low medication adherence confidence, and low perceived medication benefits. Four sub-themes related to facilitating factors were identified: structured medication habits, adequate social support, patient-physician trust and treatment confidence, and disease severity with perceived consequences. Conclusion: The factors influencing medication adherence in patients with gout are multidimensional. Healthcare professionals should provide personalized health education tailored to the specific reasons for poor medication adherence in individual patients.
