“全院一张床”模式在三甲中医医院的实践与成效

Practice and Effectiveness of the One Bed for the Whole Hospital Model in Grade A Tertiary Traditional Chinese Medicine Hospitals

  • 摘要:目的 评价“全院一张床”管理模式在三级甲等中医医院的应用成效,探索打破科室壁垒、优化医疗资源配置的有效路径。方法 采用回顾性前后对照研究,分析某市属三级甲等中医医院自2024年10月起推行“全院一张床”模式的运行数据。核心干预措施包括:建立床位统筹调度中心、推行“主管医生+托管病区”协同诊疗模式、实施中医护理同质化管理。对比分析实施前后平均待床日、床位使用率及患者满意度等核心评价指标。结果 模式实施一年间(2024年10月—2025年9月),累计跨科调配床位1518张,跨科收治患者584例。外科患者平均待床日由实施前的10.0 d缩短至3.0 d,降幅达70.00%;全院床位使用率由实施前的(87.69±7.20)%显著提升并稳定在(98.29±7.21)%(t=34.622,P<0.05),高峰期床位利用率突破100%;患者对出入院流程便捷性的满意度由92.50%(185/200)提升至98.50%(197/200),提升了6.00个百分点(x2=8.167,P<0.05)。结论 “全院一张床”模式通过集约化调度与跨科协同,在确保医疗安全与中医特色的基础上,显著提升了医疗资源的运营质效与患者就医获得感,是推动中医医院高质量发展的有效管理范式。

    Abstract: Objective: To evaluate the effectiveness of the One Bed for the Whole Hospital management model in a Grade A tertiary traditional Chinese medicine (TCM) hospital and to explore effective pathways for breaking down departmental barriers and optimizing the allocation of medical resources.Methods: A retrospective before-and-after controlled study was conducted to analyze operational data from a municipal Grade A tertiary TCM hospital following the implementation of the One Bed for the Whole Hospital model starting in October 2024. Core interventions included establishing a centralized bed scheduling center, implementing a collaborative diagnosis and treatment model of attending physician + entrusted ward, and enforcing homogeneous TCM nursing management. Key evaluation indicators, including average waiting days for beds, bed occupancy rate, and patient satisfaction, were compared before and after implementation.Results: During the one-year implementation period (October 2024 to September 2025), a total of 1,518 beds were allocated across departments, admitting 584 cross-departmental patients. The average waiting time for surgical patients decreased from 10.0 days pre-implementation to 3.0 days post-implementation, representing a 70.00% reduction. The hospital-wide bed occupancy rate significantly increased and stabilized from (87.69 ± 7.20)% pre-implementation to (98.29 ± 7.21)% post-implementation (t=34.622, P<0.05), with peak utilization exceeding 100%. Patient satisfaction regarding the convenience of admission and discharge processes improved by 6.00 percentage points, rising from 92.50% (185/200) to 98.50% (197/200) (χ2=8.167, P<0.05).Conclusion: Based on ensuring medical safety and preserving TCM characteristics, the One Bed for the Whole Hospital model significantly enhances the operational efficiency of medical resources and improves the patient experience through intensive scheduling and cross-departmental collaboration. It serves as an effective management paradigm for promoting high-quality development in TCM hospitals.
