The Clinical Effect Of The Auricular Tip Bloodletting Combined With Chaige Jieji Decoction In Treating Acute Viral Infection Fever of the Upper Respiratory Tract

• 摘要：目的 观察耳尖刺络放血联合柴葛解肌汤对急性上呼吸道病毒感染性发热患者的临床疗效。 方法 研究纳入呼吸内科及发热门诊（2024年1月至2025年1月）的90例急性上呼吸道病毒感染性发热患者，按随机数字表法分成西药组、中药组与针药结合组，每组30例，西药组予对乙酰氨基酚片进行常规退热治疗，中药组予柴葛解肌汤煎煮内服，针药结合组在中药组基础上联合应用耳尖刺络放血疗法，对比各组治疗前后的起效与完全退热时间、中医症状积分（发热、恶寒、咳嗽、咽痛、头身疼痛）、临床治疗效果、不良反应情况。 结果 与其余两组相比，针药结合组总有效率更高，治疗后退热起效与完全退热时间上更短（P＜0.05）；治疗后三组中医症状积分降低（P＜0.05），针药结合组下降程度较其余两组更为显著（P＜0.05），各组均未发生任何治疗相关不良事件。 结论 耳尖放血疗法联合柴葛解肌汤治疗急性上呼吸道病毒感染性发热疗效良好，可快速发挥退热相关通路效应，抑制机体炎症因子生成活化，调节免疫系统，兼具疗效优势与安全性。

  Abstract: Objective To observe the clinical efficacy of auricular apex bloodletting therapy combined with Chaige Jieji Decoction in patients with fever caused by acute upper respiratory viral infection. Methods A total of 90 patients with fever due to acute upper respiratory viral infection, admitted to the Department of Respiratory Medicine and Fever Clinic from January 2024 to January 2025, were enrolled. They were randomly divided into three groups (30 cases each) using a random number table: the Western medicine group, Chinese medicine group, and combined acupuncture-medicine group. The Western medicine group received conventional antipyretic treatment with paracetamol tablets. The Chinese medicine group was treated with oral decocted Chaige Jieji Decoction. The combined group received auricular apex bloodletting therapy in addition to the Chinese medicine treatment. The onset time of antipyretic effect, time to complete defervescence, Traditional Chinese Medicine (TCM) symptom scores (fever, chills, cough, sore throat, headache and body pain), clinical efficacy, and adverse events were compared before and after treatment. Results Compared with the other two groups, the acupuncture-medication combination group demonstrated a higher overall response rate, as well as shorter time to onset of antipyresis and time to complete fever resolution (P<0.05). After treatment, the TCM symptom scores were all reduced in the three groups(P<0.05). The reduction was more pronounced in the acupuncture-medication combination group than in the other two groups (P<0.05). No treatment-related adverse events occurred in any of the groups. Conclusion Auricular apex bloodletting therapy combined with Chaige Jieji Decoction is effective in treating fever due to acute upper respiratory viral infection. It can rapidly exert antipyretic pathway-related effects, inhibit the production and activation of inflammatory cytokines, regulate the immune system, and has both efficacy advantages and safety.
