Meta-analysis of the therapeutic effect of superficial palmar branch of radial artery flap and venous flap in repairing soft tissue defects of fingers

• 摘要：目的 使用Meta分析评价桡动脉掌浅支皮瓣(Superficial palmar branch of radial artery flap, SPBRAF)与动脉化静脉皮瓣(Arterialized venous flap, AVF)用于修复手指软组织缺损的临床疗效，分析两者现有临床研究的证据，为临床医师行手指软组织缺损修复时提供选择参考。 方法　检索知网、万方、维普数据库、PubMed、Embase、Cochrane Library、Web of science 2000年1月1日至2024年6月30日有关SPBRAF与AVF修复手指缺损疗效比较的文献，按照纳入排除标准筛选并提取文献，使用RevMan5.4软件和Stata17.0软件进行Meta分析，以皮瓣存活率、手术时间、手指活动度、手指感觉恢复情况、并发症发生率作为结局指标。 结果　本研究共纳入10 项研究，共包含539个病例，其中272个病例使用SPBRAF修复手指缺损，267个病例使用AVF修复手指缺损。Meta分析结果显示： SPBRAF组皮瓣存活率更高OR:4.37,95%CI:（2.03,9.41）,Z=3.77,P=0.0002，手指活动度更好OR:5.58,95%CI:（2.71,111.48）,Z=4.66,P＜0.00001，手指感觉恢复更好MD:-0.96,95%CI:（-1.52,-0.39）,Z=3.33,P=0.0009、OR:5.58,95%CI:（2.71,111.48）,Z=4.66,P＜0.00001，并发症发生率更低OR:0.48,95%CI:（0.24,0.95）,Z=2.12,P=0.03。AVF组手术时间更短MD:14.88,95%CI:（7.10,22.66）,Z=3.75,P=0.0002。差异具有统计学意义（P＜0.05）。 结论　SPBRAF和AVF均可在临床上用于手指软组织缺损的修复，但SPBRAF在皮瓣存活率、手指活动度、感觉恢复情况、并发症发生率这些方面表现的更加优秀，AVF在手术时间上更占优势。未来仍需要更大样本、更高质量的临床试验全面进一步评估。

  Abstract: Objective To evaluate the clinical efficacy of superficial palmar branch of radial artery flap (SPBRAF) and arteriovenous flap (AVF) in repairing finger soft tissue defects through a Meta-analysis, and to analyze the existing clinical research evidence to provide a reference for clinical physicians when choosing a flap for finger skin defect repair. Methods Relevant literature comparing the efficacy of SPBRAF and AVF in repairing finger defects from January 1, 2000 to June 30, 2024 was retrieved from PubMed, Embase, Cochrane Library, Web of Science, CNKI, Wanfang, and VIP databases. The literature was screened and extracted according to the inclusion and exclusion criteria, and a Meta-analysis was conducted using RevMan 5.4 software. The outcomes included flap survival rate, operation time, finger range of motion, finger sensory recovery, and complication rate. Results A total of 10 studies were included, involving 539 cases. Among them, 272 cases were repaired with SPBRAF and 267 cases with AVF. The Meta-analysis results showed that the flap survival rate was higher in the SPBRAF group OR: 4.37, 95% CI: (2.03, 9.41), Z = 3.77, P = 0.0002, finger range of motion was better OR: 5.58, 95% CI: (2.71, 111.48), Z = 4.66, P < 0.00001, finger sensory recovery was better MD: -0.96, 95% CI: (-1.52, -0.39), Z = 3.33, P = 0.0009, OR: 5.58, 95% CI: (2.71, 111.48), Z = 4.66, P < 0.00001, and the complication rate was lower OR: 0.48, 95% CI: (0.24, 0.95), Z = 2.12, P = 0.03. The operation time was shorter in the AVF group MD: 14.88, 95% CI: (7.10, 22.66), Z = 3.75, P = 0.0002. All differences were statistically significant (P < 0.05). Conclusion Both SPBRAF and AVF can be used in the clinical repair of finger soft tissue defects, but SPBRAF has better efficacy, while AVF is more convenient. Future studies with larger sample sizes and higher quality are still needed for comprehensive and further evaluation.
